To Whom It May Concern
My name is Awatif S. Attalla and I had health insurance with your company since April 10, 2012. My policy number was 61180827 and the policy covered me, my daughter (Marina Attalla) and my husband (Mamdouh Attalla). An automatic withdrawal from my checking account was setup for monthly withdrawal.
On Oct 2012, I spoke to Latoya to cancel Awatif and Marina health plan but to keep only my husband (Mamdouh Attalla) on the policy. Accordingly Latoya informed me that as of Oct 10, 2012 midnight me and my daughter policy will be cancelled and only my husband will be on it and she gave me a new policy number 0061368132. And the new policy took effect on Oct 10, 2012 midnight.
On August 6, 2014 morning I called to give you my new bank account information because I changed the bank account to continue withdrawal of the premium for my husband health insurance. Andrew informed me that your insurance company has been withdrawing the whole amount for all three of us and he said that he has to transfer my call to Melissa to take care of that issue.
Melissa who is responsible for issuing new polices told me that it is showing on her records that I called on Oct 10, 2012, requested the cancellation of me and my daughter policies, and keep my husband policy. In addition, she explained to me that she could not help me because she only issues new policies. Therefore, she transferred me to Policy Holder Service department and I spoke to Melissa the supervisor, she asked me to write a letter regarding this matter.
Therefore, I am requesting a refund of all the premiums that was withdrawn from my bank account erroneously for me and my daughter Marina from the date of Oct 10, 2012 through now. And just keep my husband (Mamdouh Attalla) on his policy.
